• The Ohio Conference of Teamsters Insurance becomes active immediately following your probationary period, usually 60-90 days. Once the fund begins receiving contributions, insurance cards and enrollment forms will be mailed directly to you.
   
• Important: The enrollment form must be filled out and returned immediately in order to cover your dependents.
